AutumnBridge Hospice understands that a life-limiting illness creates a number of challenges. These challenges can range from physical and emotional pain to caregiving and financial burdens.

Our individualized care plan includes:
• Specialized nursing care
• Physician services
• Personal care provided by certified home health aides
• Medical social services
• Pain control and symptom management
• Spiritual counseling
• Massage therapy
• Continuous care during short-term periods of crisis
• Other therapeutic services as needed
• Volunteer companionship and support
• Prescriptions related to the hospice diagnosis
• Pain medication
• Medical equipment and supplies related to the hospice diagnosis
• Nutritional management
• Family education and caregiver support
• Respite care for family or caregiver relief
• Assistance with funeral arrangements
• Grief and bereavement support for 13 months
In addition, our care team is available to help 24 hours a day, 365 days a year.
