AutumnBridge Hospice makes the following commitment to our patients:

• To treat you as a whole human being—body, mind, and spirit.

• To maintain hope and encourage your loved ones to also maintain a sense of hopefulness.
• To answer your questions about your care clearly and honestly.
• To give you the opportunity to participate in decisions concerning your care and to give you as much control over your life as possible.
• To allow you to express your feelings about your illness and to enable you to approach death in your own way.
To respect your choices concerning the way you prefer to die and your choice concerning the disposition of your body.
• To help you live as pain-free as possible.
• To offer your loved ones help (if needed) in dealing with your illness and death.
• To listen to your spiritual beliefs and experiences and to provide spiritual support.
• To provide competent and sensitive professionals who will seek to understand and help you with your needs and desires.
• To help you to live as fully as possible for as long as possible.
• To be with you until the end.

Our hospice team, both professionals and volunteers, have selected this profession because we believe it is a special privilege to be a part of hospice. We find joy and satisfaction in our work, and we want to share these feelings with others.

Our Services
Our Approach to Care

AutumnBridge Hospice offers a comprehensive, team approach to providing care in a patient’s home or homelike setting. We believe that each patient should be able to live in comfort and with dignity, and that families should receive the necessary support to allow their loved ones to do so.

Our philosophy of care affirms life and accepts death as a natural part of life. Our services neither hasten nor prolong the dying process. Our focus is to make our patients’ final months as comfortable and enjoyable as possible.
